Your Make-up Report will be forwarded to Attendance Chair
to credit your attendance and to Club Treasurer who will credit $15.00
to your account.
Do not make out a blue Make-Up Report form if you
submit this form electronically. Thank you for taking time to submit a Make-up Report.
